GDV can be life threatening if not treated quickly. Often times the clinical signs alone can tell you that it is GDV. However an x-ray will give a definitive diagnosis. It is important to figure out whether the stomach is just bloated or if the stomach is twisted. The best way to figure this out is an x-ray. It will let you differentiate between the two and decide if surgery is needed.
However, if a patient is not stable then an x-ray should not be the first thing done. The first thing done should be to stabilize the patient. Once the patient is stabilized, then an x-ray can be done for diagnosis.
