I've worked with three Shetland Sheepdogs in my lifetime. These dogs have a very good temperament and are very friendly. They don't bark too much, although I have had the experience that they will bark at their owners when they leave the house, just a minor bit of separation anxiety. It wasn't really a big deal. They are okay for inside or outside dogs as far as size in my opinion. The only problem you might have with them being inside is their hair and the issue of shedding. They have A LOT of hair and shedding can become a problem. Also due to their thick hair, a warm climate may not be the best option for their level of comfort. These dogs work very well with children. They are gentle and I've only dealt with one that was even slightly aggressive. Their size is not a problem when it comes to children because they are fairly low to the ground. As far as health problems, I've never had an issue with that as far as these dogs are concerned. The ones I've dealt with died of old age/ natural causes.
